Report an Incident
Click the Report an Incident button on the right to report an issue impacting your account, a system, or an IT service.
- An issue is when something is broken or not working correctly.
Submit an IT Request (a.k.a Service Request)
Use the Submit an IT Request form for IT requests such as account changes, access requests and software/hardware installs.
- A service request is a request for information, advice, a request for a change, or a request for enhancements.
Notes
- If you are experiencing more than one issue, it is strongly recommended to create a separate ticket for each incident as tickets are assigned to different IT staff depending on the nature of the ticket.
- When reporting an incident or requesting service, please provide as much detail as possible. It will be helpful for us to know the type of device or application that you are experiencing issues with.
- Please do not include personal/confidential information in web forms. This information includes, but is not limited to:
- social insurance numbers (SIN)
- passwords
- birthdays
- any other information you would not share with a stranger
